Purpose: Inguinofemoral lymphadenectomy (IFLA) is a standard procedure for cancer of the external genitalia. Open lymphadenectomy (O-IFLA) exhibits complication rates of more than 50%. We are demonstrating our extended experience with a modified endoscopic approach (E-IFLA) for groin lymphadenectomy.

Patients and Methods: Patients with nonpalpable as well as those with palpable nodes who had IFLA were identified. O-IFLA comprised both superficial and deep inguinal lymph node dissection. E-IFLA was performed using a three-trocar approach in the same field. We used a reduced CO2-pressure of <5mm Hg. A suction drain was always placed. Perioperative data and postoperative outcomes were systematically assessed followed by statistical analysis.

Results: We performed 62 IFLAs in 42 patients. Twenty-eight procedures were completed endoscopically. Follow-up was 55.8 months (2-87 mos). Mean operative time for O-IFLA was 101.7 minutes (38-195 min), being shorter than for E-IFLA (136.3 min, 87-186 min), P < 0.001. Both groups are comparable regarding the number of nodes (O-IFLA 7.2, 2-16 vs E-IFLA 7.1, 4-13) as well as with regard to the number of positive nodes (O-IFLA 1.8 vs E-IFLA 1.6). Secondary wound healing and leg edema were extremely rare events (1/28) after E-IFLA. The overall complication rate was 7.1%. Complications appeared in 55.3% of the O-IFLA-cases. Batch studies were conducted to study the effects of dose, pH, and equilibrium time on 4-CP removal. To improve 4-CP removal, surface modification of the adsorbent with TiO(2), HNO(3), and/or NaOH was undertaken.

RESULTS: At an initial 4-CP concentration of 25 mg L(-1) under optimized conditions (dose 13.5 g L(-1), pH 2.0; agitation speed 150 rpm and 50 min equilibrium time), the NaOH-treated CSC demonstrated a greater removal of 4-CP (71%) than those oxidized with HNO(3) (40%) and/or coated with TiO(2) (52%). The adsorption capacity of the NaOH-treated CSC (54.65 mg g(-1)) was higher than those treated with HNO(3) (23.13 mg g(-1)) or coated with TiO(2) (48.42 mg g(-1)).

New insights into the pathophysiology of trauma-induced coagulopathy, the increasing availability of point-of-care devices and awareness of side effects of intravenous fluids and traditional fresh frozen plasma therapy has encouraged new concepts for managing massive blood loss.

Recent findings

Trauma-induced coagulopathy primarily results from

blood loss, hypovolemia-induced activation of the protein C system and consequent increase of the fibrinolytic potential, whereas hemodilution, localized consumption of clotting factors and platelets, hypothermia, acidosis, anemia and hypocalcemia further decrease the hemostatic potential. The widespread use of viscoelastic devices highlighted the importance of the contribution of fibrinogen to clot firmness, a precondition for cessation of bleeding. The evidence is growing that targeted therapy using coagulation factor concentrates guided by viscoelastic measurements enables effective correction of severe coagulopathy.

Summary

During massive blood loss, viscoelastic measurements should guide aggressive treatment of deficiency or hyperfibrinolysis or both. In addition, the impact of contributing factors should be considered and as far as possible corrected.

The Self-Administered Leeds Assessment of Neuropathic Symptoms and Signs (S-LANSS) is a 7-item self-report scale developed to identify pain of predominantly neuropathic origin. The aim of this study was to develop a Turkish version of the S-LANSS and to test its validity and reliability in chronic pain patients.

Method and Patients.

We enrolled 244 chronic pain patients treated at the Neurology Department. The original version of the S-LANSS was translated into Turkish by standard procedures. An independent clinician determined the pain type (neuropathic vs nociceptive). The reliability (internal consistency and test-retest

reliability) and validity (agreement with the reference diagnosis and sensitivity, specificity, and positive and negative predictive values) were determined.

Results.

Two-hundred and forty-four Epacadostat datasheet patients with chronic pain (167 women, 43.1 +/- 11.4 years), 137, neuropathic pain and 107, nociceptive pain, were asked to complete the S-LANSS twice. Cronbach’s alpha-coefficient was 0.74 for the test and 0.73 for the retest. Total S-LANSS scores for subjects did not significantly differ between applications (P = 0.46).

Correlation coefficient TH-302 mouse was r: 0.97 (P < 0.01), which is fairly high for a self-assessment tool. a retrospective analysis of acute bacterial prostatitis (ABP) to evaluate the factors of progressing to chronic infection and chronic pelvic pain syndrome IIIa (CPPS IIIa) from ABP. The clinical records of 480 cases compatible with a confirmed diagnosis of ABP from five urological centers between 2001 and 2010 were reviewed. We defined chronic infection (CI) as a progression to chronic bacterial prostatitis (II), epididymo-orchitis, and showing persistent pyuria and bacteriuria after treatment of ABP in admission periods when followed up at 3 months or more. Results were analyzed according to two categories: category I, developed to CI (group A, n = 49) versus recovered without CI or CPPS IIIa (group C, n = 385); and category II, developed to CPPS IIIa (group B, n = 46) versus recovered without CI or CPPS IIIa (group C, n = 385). Of the 480 ABP patients, 10.2% (49/480) progressed to CI and 9.6% (46/480) progressed to CPPS IIIa. The frequency of CI was 11.3% (49/434) and that of CPPS IIIa was 10.7% (46/431).

METHODS: In a cross-sectional longitudinal study, individual TB case variables were identified from three sources-routine surveillance, laboratory and hospital discharge records-from 1997 to 2007. Community variables were obtained at the census tract level. A deprivation index was calculated based on percentages of unemployment, low educational level and unskilled labour. Multilevel Poisson models were estimated for TB incidence rates for patients with and without HIV.

RESULTS: A total of 490 TB cases were included. Sex and age at individual level and deprivation and residence in the port area at census tract level were associated both with HIV-related TB and with non-HIV-TB. Household crowding contextual variables were also associated with HIV-related TB incidence. Full models account for 78.9% and 51.7% reductions in second-level variance.

CONCLUSION: Socio-economic deprivation is associated with higher rates of HIV-TB and non-HIV-TB. Diverse individual and contextual potential risk factors suggest different pathways of transmission. It is necessary to extend the framework for intervention beyond individual-based strategies to the socio-economic contexts in which people live.”

“Gastrointestinal stromal tumors (GISTs) of the rectum are rare, and their clinical manifestations are variable. We report the case of a

23-year-old Bafilomycin A1 order woman who was referred complaining of dyspareunia during coitus. We performed digital vaginal and rectal investigations, ultrasound, colonoscopy, and computed tomography. These examinations confirmed the presence of a lesion measuring about 6 x 5 x 5 cm in size attached to the posterolateral left rectum wall. We performed a circumanal excision, then arrived at the mass between the muscles of the pelvic floor. Pathological studies indicated that the tumor was a high-grade GIST, and was clear at the surgical margins. On follow-up, a GIST was found at the gastric fundus via an endoscopic biopsy. Imatinib treatment was initiated because the patient refused a second operation. The gastric lesion disappeared 6 months after imatinib treatment. Dyspareunia disappeared 1 month after operation.
